Abstract: Due to an absence of effective adjuvant treatment for renal cell carcinoma, only surgery offers a possibility for cure. Radical nephrectomy of localized disease is the gold standard. Some observations have shown benefit from of extended regional lymph node dissection. Conservative nephron-sparing surgery in unilateral disease is still controversial and should be reserved only for peripheral solitary tumousr with a diameter less than 3.5 cm. The removal of caval thrombi could improve the survival of patients. Also the removal of resectable distant solitary metastasis can improve the prognosis in some cases.
