Carcinoma cervici suteri - sixty years-treatrnent period

Sumary:During the course of 60 years, i.e. from 1938 to 1998 inclusively a total of 7270 patients with carcinoma of the uterine cervix were treated at the Masaryk Memorial Cancer Institute in Brno. Of these, 6339 patients, ie. 87,2 % were irradiated, 931 i.e. 12,8 % underwent primarily a radical operation. The mean age was 50,2 years, the fifth decade beeing numerically the highest in incidence - 24,.0 %. Carcinoma was diagnosed and classified precisely according to valid international directions-FIGO quidelines. In addition to basic examination, limb lymphography and CT scan were intrroduced, giving the strategy of radical irradiation. when lymphatic system is involved and in early stages of the disease. The prognosis of this carcinoma defined by clinical stage, histological type and grade was objectively expressed by the evaluation of therapeutic results. The change of the technique in the therapy of this carcinoma based upon a precise pretreatment planning increased the percentage of cured patients. The complexity of the therapy, the observance of therapeutic plans and improved dosimetry are the perrequisites not only for the individualization of therapeutic procedures but also for the achievement of the best therapeutic results. In spite of a high dose summation given by the extent of megavoltage irradiation, the number of postirradiation reaction, nor early late, is increased not to much. The extension of chemotherapy and the more detailed analysis of the effect of chemotherapeutics stimulated the potentiation of radiotherapy by suitable chemotherapy used simultaneously. A comparison of the results of treatment in patients. radically irradiated- 671 and those primarily operated upon, i.e. in 850 patients of stage I. showed the 5-year recovery to be superior by almost 5,0 % ( 83,4 % against 78,4 % ). An increasing mortality rate due to carcinoma is noted with advancing stage, age, and grade of carcinoma ( mainly in the last 6-8 years).
