EXPRESSION OF TUMOR INFILTRATING T LYMPHOCYTES IN RECTAL ADENOCARCINOMA TREATED BY ADJUVANT POSTOPERATIVE RADIOTHERAPY

Purpose: To evaluate the expression of tumor infiltrating T lymphocytes (TIL) in locally advanced rectal cancer treated by postoperative adjuvant radiotherapy.
Patients and Methods: Between August 1995 to July 2002, 19 patients (pts), 9 males and 10 females, median age 63 (42-80) years, with rectal adenocarcinoma were treated. All pts had histology of adenocarcinoma: 3 pts grade 1, 9 pts grade 2 and 7 pts grade 3. Pathologic TNM stage was as follows: 11 pts stage II and 8 pts stage III. Anatomic sublocalization were as follows: 6 pts low rectum (0-5 cm), 4 pts middle rectum (5-10 cm) and 9 pts upper rectum (upper than 10 cm). Adjuvant postoperative radiotherapy follows after R0 resection in 17 pts and after R1 resection in 2 pts. Radiotherapy consisted of external beam radiotherapy 44 - 60 Gy in 22 - 30 fractions. In all pts were immunohistochemically investigasted TIL, EGFR, VEGF and p53. The number of CD3 positive TIL in tumor tissue was evaluated quantitatively (number of lymphocytes in 1 mm2 of tumor) and the expression of EGFR, VEGF and p53 semiquantitatively.
Results: The median of pretreatment hemoglobin was 123 (97-144) g/l, leucocytes 8 (4.38-11.3) 109/l, thrombocytes 306 (126-467) 109/l and CEA level 1.51 (0.19-62.9) ěg/l. At the date of evaluation (March 31st, 2006) 8 pts were alive. Seven pts had recurrence: 2 local, 4 distant metastases and 1 pt synchronous local recurrence and distant metastases. Median percentage of tumor cells membraneous and cytoplasmic EGFR positive was 3%, mean 20%, range (0-80%), median grade of membraneous positivity of EGFR was 2, mean 1.3, range (0-3), median percentage of tumour cells membraneous EGFR positive was 1%, mean 10%, (range 0-70%), median percentage of tumor cells cytoplasmic EGFR positive was 0%, mean 16%, (range 0-65%), median percentage of VEGF expression was 0%, mean 92%, range (50-100%), median percentage of p53 was 60%, mean 48%, range (0-100%). Median number of TIL in 1 mm2 was 488, mean 542, range (69-1250).
Conclusions: The results of this pilot study demonstrates the expression of TIL in locally advanced rectal cancer treated by postoperative adjuvant radiotherapy. Acknowledgement: Supported by the research project MZO 00179906 and Terry Fox Foundation.
