Summary: Principal morphological and subcellular parameters for differential diagnosis, stratification and prognostication of soft-tissue sarcomas are reviewed. Surgical methods are envisaged in their limitations and weak points in order to stress the need for combined therapeutic approaches. Pre-operative intraarterial chemotherapy, intersticial brachytherapy and individualized chemotherapy based on predictive testing may represent few newer approaches to enhance still rather low effectivity of the treatment of soft-tissue sarcomas.
