Summary: The role of radiotherapy was several times reevaluated in the history of the treatment of the breast cancer. New clinical trials brought a strong evidence not only for the reduction of lokoregional recurrences after breast conservative surgery in the early breast cancer and after mastectomy in the locally advanced disease, but also for the improvement of overall survival. The complications were significantly reduced with new techniques of irradiation. Adequate equipment and precise treatment are the basic condition for the benefit of radiotherapy.
