Therapy of advanced stages and relapses of germ cell testicular tumours

Abstract: The author discusses improvements in primary chemotherapy for stages nC, III and IV testicular cancer, mainly after introduction of cisplatin into clinical practice. However, an optimal therapeutic strategy is not given. Postchemotherapy surgery is valuable for obtaining tissue for histological examination and to aid in the decision concerning further treatment, especially in the case when tumour viability is persistent. The tendency for selection of patients into ´good prognosis´ and ´poor prognosis´ groups is well known. Minimalization of therapy due to side effects is recommended for patients with good prognosis. In patients with poor prognosis (chemoresistance on the basic scheme, early relapses) aggressive chemotherapy (high-dose chemotherapy with myeloprotection) is recommended. In advanced stages of seminoma primary chemotherapy with carboplatin containing regimen is indicated.
