Abstract: Voice quality after primary radiation therapy of carcinoma of the larynx is generally superior to the quality after primary surgery and survival is similar in patients whether they recieve operative treatment or radiotherapy (stage I, II). Fractionation of irradiation dosage was one the most important steps forward in the early years of radiotherapy. From 1981 to 1986, 190 patients who were diagnosed with squamous cell carcinoma of the larynx weretreated and included in this study. Patients were non-randomly allocated to receive either - Group A - Co60,64 Gy in 32 fractions, Group B - Co60,64 Gy in 64 fractions (2,0 Gy per day in two fractions). No significant differences were seen between groups A and B. The disease-free survival for all patients was 55% (5 y) and 36% (7 y). The locoregional control rate for all patients was 49% (5 y) and 33 % (7 y).
